Notes:
- The use of antiplatelet therapy, thrombolytics and anti-thrombin treatment in patients with unstable angina and NSTEMI are covered in NICE CG 94: Unstable angina and NSTEMI
- Bivalirudin is indicated for the treatment of STEMI in patients undergoing PCI when used in accordance with NICE TA 230: Bivalirudin for the treatment of STEMI (Jul 2011)

Prophylaxis for VTE prevention should continue for 14 days post knee revision and 35 days post hip revision. - NICE CG 144: The management of venous thromboembolic diseases gives guidance on the care of adults with VTE
